Change of Credential Application

In completing this application the applicant is applying for a change in credential status from “Licensed” to “Ordained.” The application must be completed in full. Please ensure that you provide your full name in the space provided: first, middle, and last (no initials).

An application fee is payable and must be included with this application.

A copy of your “Ordination Certificate” or a letter of attestation from your ordaining minister must be included with the application as well.

Licensing and Ordination Policy
    1. Once this change in status is approved, we can apply to you Vital Statistics office in your province to register you for the solemnization of marriages. Should you desire this registration you must advise our office as it is not automatically done for you.
